What You Should Be Able to Explain After Reading
- Assess functional impact and relevant alternatives.
- Progress loading across length and speed demands.
- Use running exposure as a rehabilitation variable.
- Base return to sport on demonstrated readiness.
Assessment and differential diagnosis
- Document mechanism, location, bruising, walking/running tolerance and previous injury.
- Assess pain-limited range, strength and functional tasks when appropriate.
- Consider proximal tendon injury, lumbar referral, neural involvement and major avulsion when the pattern is atypical.
Rehabilitation progression
- Restore comfortable gait and range, then introduce tolerable isometric and isotonic loading.
- Progress toward higher-load and longer-muscle-length strength work.
- Introduce running when basic loading and low-speed drills are tolerated.
- Progress from linear submaximal running toward acceleration, deceleration, sprinting and sport-specific exposure.
Dosage and reassessment
- Strength work can use 2–4 sets across moderate repetition ranges 2–3 days per week, individualized to stage and schedule.
- Increase one major running variable at a time when possible: volume, speed or complexity.
- Reassess symptoms, strength, sprint exposure, confidence and next-day response.
Common mistakes and clinical pearls
- Pain-free jogging is not a final return-to-sport criterion.
- Do not chase flexibility while neglecting force production.
- Clinical pearl: prepare the athlete for the fastest and most demanding tasks they will actually face.
Summary
Hamstring rehabilitation should progressively rebuild force and running speed, with return to sport based on criteria rather than time alone.
