EP 30Dizzy Standing OvationGeriatric and Cardiovascular Safety
The Dizzy Standing Ovation
Dr. SajjadWhy is the older patient sitting back down after standing?
InternThey felt light-headed, Sir. I told them to stand again quickly so their body could get used to it.
Dr. SajjadDid you support them and check whether the symptoms settled?
InternNo, Sir. I thought standing practice means more standing.
Dr. SajjadNot when the patient is dizzy. Help them sit or lie safely, prevent a fall and assess the symptoms. If indicated and you are trained, check lying and standing blood pressure using the correct protocol. Consider orthostatic hypotension, but do not diagnose from one symptom.
InternShould I repeat the standing test until they stop feeling dizzy?
Dr. SajjadNo. Stop provoking the symptom. Review the situation, follow escalation guidance and only resume mobility when it is safe.
Clinical takeaway
Light-headedness or dizziness after moving to sitting or standing may be related to orthostatic hypotension, but other causes must also be considered. Assist the patient to a safe sitting or lying position, prevent falls and assess symptoms. Measure postural blood pressure only when indicated and according to a correct protocol by a trained clinician. Escalate persistent, severe or concerning symptoms and do not repeatedly provoke dizziness to complete a mobility task.
