Physio Nexus Academy · Clinical Treatment Framework

Carpal Tunnel Syndrome

Education, activity modification, night splinting and selected nerve/tendon gliding can help mild-to-moderate cases; progressive motor loss requires medical review.

Clinical decision pathway

A structured plan, not a recipe

Clinician-led · Individualised
01

Understand

Screen safety, identify the likely clinical pattern and agree meaningful goals.

02

Select

Choose only interventions that match the patient's impairments and preferences.

03

Reassess

Repeat the same comparable sign or functional task after key interventions.

04

Progress

Build capacity and participation using measurable response, not session count.

Before choosing an intervention

Clinical reasoning

  1. 1Mild-to-moderate cases may respond to night splinting, activity modification and nerve/tendon gliding.
  2. 2Progressive thenar weakness or constant major sensory loss needs medical review.
  3. 3Screen cervical/proximal median nerve contribution.

A practical starting sequence

First-session workflow

  1. 1Record symptom distribution, thenar strength, grip/pinch and functional limits.
  2. 2Assess provocative positions and work habits.
  3. 3Fit/advise neutral wrist positioning at night when indicated.

Assessment & examination

  1. 1Median-nerve sensory symptoms
  2. 2Thenar strength
  3. 3Provocative tests
  4. 4Cervical/proximal nerve screen
  5. 5Work and repetitive-load factors

Red flags & safety screen

  1. 1Progressive thenar weakness/atrophy
  2. 2Constant severe numbness
  3. 3Traumatic onset or systemic neurological disorder

Indication · dosage · response

Manual therapy application

  1. 1Median nerve sliders should remain gentle and should not create prolonged numbness.
  2. 2Carpal/forearm mobilization can be trialed if it improves symptoms.
  3. 3Avoid aggressive pressure directly over the carpal tunnel.

Adapt to irritability and capacity

Exercise prescription & dosage

  1. 1Median nerve slider: 5–10 gentle reps, 1–3 times/day initially.
  2. 2Tendon glides: 5–10 reps, 2–4 times/day.
  3. 3Grip/hand strengthening later: 2–3 sets × 8–15 reps.

Modalities & adjuncts

  1. 1Night neutral wrist splinting is often more useful than electrotherapy for symptom control.

Progression & regression rules

  1. 1Increase hand loading as nocturnal symptoms and irritability reduce.
  2. 2Progress work tasks gradually.
  3. 3Escalate if strength declines.

Home programme

  1. 1Reduce sustained wrist flexion/extension
  2. 2Night splinting when indicated
  3. 3Frequent movement breaks

Outcome measures

  1. 1Boston Carpal Tunnel Questionnaire
  2. 2Grip/pinch strength
  3. 3Sensory symptoms

Common clinical mistakes

  1. 1Repeated strong nerve stretching.
  2. 2Ignoring thenar weakness.
  3. 3Using only wrist massage.
  4. 4Keeping the wrist splint on all day without need.

Patient education & self-management

  1. 1Avoid long sustained wrist flexion/extension when possible.
  2. 2Use frequent micro-breaks for repetitive work.
  3. 3Explain when surgical/medical opinion may be needed.

Referral & escalation

  1. 1Medical/hand-surgery review for progressive weakness, atrophy or persistent significant sensory loss

Clinical use & safety note

This is an educational clinical framework, not an individual prescription or a substitute for diagnosis, medical assessment, local protocols or professional judgement. Exercise dose, manual techniques and adjuncts must be adapted to examination findings, irritability, comorbidities, precautions, patient goals and response. Obtain informed consent, work within training and scope, and refer when indicated. Do not apply every intervention listed to every patient.

The PNA treatment principle

The best plan is not the one with the most techniques. It is the one that identifies the patient's main problem, selects a defensible intervention, measures the response and steadily restores meaningful function.
